What semaglutide is
Semaglutide is a GLP-1 receptor agonist: it imitates the gut hormone GLP-1 to reduce appetite, slow how fast the stomach empties and improve blood-sugar control. It was developed by Novo Nordisk for type 2 diabetes, and its powerful effect on appetite made it a landmark weight-loss medicine. Learn the biology in how GLP-1 works.
Brands & forms in South Africa
- Ozempic — weekly injection, registered for type 2 diabetes; used off-label for weight loss.
- Wegovy — weekly injection at higher doses, registered for chronic weight management (launched in SA in 2025).
- Rybelsus — a once-daily oral semaglutide tablet, registered in SA but not yet marketed here as of 2026.

How well semaglutide works
In the STEP 1 trial, semaglutide 2.4 mg produced about 15% average weight loss over 68 weeks in adults without diabetes. It's second only to tirzepatide on average weight loss — see semaglutide vs tirzepatide. Results vary between individuals.
Side effects
Mostly gastrointestinal (nausea, constipation, diarrhoea), worst when starting or increasing dose. Rare serious risks include pancreatitis and gallbladder problems, with thyroid-cancer cautions. Full detail in our side effects guide.
